Original PSA Birth Certificate
Original copy of Form 137-A

Four (4) 2x2 I.D. Pictures white background

Certificate of Good Moral Character
Original PSA Birth Certificate

Transferees

Transfer credentials and copy of subjects taken from the previous school.
Certificate of Good Moral Character.
Original PSA Birth Certificate.
Four (4) 2x2 I.D. Pictures white Background.
The student transferee should undergo interview with the Dean/ Program Head, Discipline Officer and must sign a Behavioral Contract.
